RN Care Manager (Full-Time, Remote, Must Reside in WY or Bordering State)
The RN Care Manager (Full-Time, Remote, Must Reside in WY or Boarding State) is responsible for providing case management services to high acuity clients with complex physical, social, and behavioral health conditions. This position works closely with the Mountain Pacific, state Medicaid, contractor leadership and provider agency staff to ensure program requirements are met.
In addition to the above, this position is also expected to work as a team with the Care Management staff to remain in compliance with cases and contract deliverable expectations on a daily basis.

The RN Care Manager position is a full-time remote position in Wyoming or bordering state. The successful candidate would have excellent written and verbal communication, organizational skills, and attention to detail. A current licensure in the State of Wyoming with a minimum of 4 years' experience in case management, clinical, hospital, nursing home, or home health preferred. The target compensation for this position is $68,000 - $78,000 a year. The first review of candidates will be on November 25, 2024, and continually reviewed every 5 days after.
